  1. You have not been hacked.
  2. This seems to rarely happen on a very small number of iPhones.
  3. The most likely culprit is you've installed additional emojis on your iPhone via an App and one or more emojis are replacing icons that are displayed in web pages in Safari. To fix remove the App.
